How can I submit a name to the registry?

You can submit a name of a veteran at no cost by printing and completing this form. You may also contact us for a print version.

In addition, you may submit 100 words or less of biographical information that will be included for a one-time fee of $25. A photo is an additional $25, and should be no larger than 4 x 6. Photos will be returned.
